Begin by placing the chicken breasts at the bottom of your crockpot. They will act as a base and infuse the broth with flavor as they cook.
Pour in the chicken broth over the chicken, ensuring they are submerged.
Add the diced tomatoes, minced garlic, diced onion, and Italian seasoning into the pot. Stir gently to combine the ingredients.
Cover the crockpot and cook on low for 6-7 hours or on high for 3-4 hours until the chicken is cooked through and tender.
Once the chicken is cooked, carefully remove it from the crockpot and shred it using two forks. Return the shredded chicken back to the pot.
Stir in the frozen tortellini and heavy cream, mixing well. Cover the crockpot again and cook on high for an additional 30 minutes, or until the tortellini is cooked through and tender.
Before serving, taste and season with salt and pepper as needed.
Serve hot, garnished with fresh basil leaves and a sprinkle of grated Parmesan cheese.
